FREEDOM OF INFORMATION ACT REQUEST (click link for form):  Freedom of Information Act request forms are used to obtain copies of reports, photographs or other documents generated by the Negaunee Police Department on investigations other than UD-10 Crash Reports.

  1. In the space provided at the top of Page 1, indicate specifically what you are looking for.  The information you give will dictate what is compiled for you.  If you have dates and/or names please include them here.
  2. Just under this area is where you will provide your personal and contact information.
  3. The bottom half of Page 1 is for office use; however, you may want to read it over as you are responsible for charges incurred by the compilation of the documents you request.  Page 2 should also be reviewed before you submit your request.
  4. Return the form to the Negaunee Police Department in person.  If you cannot come to the Police Department in person, please call (906)475-4154 to inquire about arrangements to fax or mail.
  5. You will be contacted within 5 business days to either pick up your documents (via telephone) or notified your request was denied (via mail).  If you request is denied you will be supplied with the reason and alternate methods of obtaining the documents if available.

BICYCLE REGISTRATION (click link for form):  Bicycles belonging to residents of Negaunee can be registered free of charge with the Police Department.

  1. After printing out the form, fill in the information spaces as completely as possible, leaving the "License Number" space blank as this will be assigned upon receipt by the Police Department.  The serial number (or Owner Assigned Number) is mandatory for identification purposes.
  2. Once the form is complete, return it to the Police Department in person.  At that time you will receive a license sticker and an informational sheet for the bicycle.

NSF OR NO-ACCOUNT CHECKS (click link for form):  You must fill out one set of paperwork for each bad check even if the same person passed multiple checks.  Each is a separate offense and needs separate forms completed.

  1. Fill out the two identical sections of Page 1 with the same information.  Cut the bottom section off and mail to the passer of the bad check.
  2. Fill out Pages 2 & 3 as completely as possible and attach the top section of Page 1 to them.   Wait for five days after mailing the bottom section of Page 1 before proceeding to Step #3.
  3. If after five days you do not receive satisfaction in the form of payment in full* or make arrangements with the passer for payment, bring the paperwork AND the bad check itself to the police department for further action.

*Note: If you accept partial payment for any NSF/No-Account check, the Police Department will not initiate a criminal complaint on the matter.  By accepting partial payment you will have entered into a civil agreement.

PARADE PERMIT or STREET USE PERMIT (click link for form):  After printing the form complete all fields.  Sign the form and bring it to the Police Department for review.  If your application is approved you will receive a signed copy in the mail.

CITY COMPLAINT FORM (click link for form):  After printing the form complete all fields.  Bring the completed form to either the City Manager's Office or the Chief of Police for review and further action.
